Proof of Identification

Under the Immigration, Asylum and Nationality Act 2006 Armstrong Appointments are required to obtain and copy documentation proving an employee’s right to work in the UK.

Please provide proof of your right to work in the UK and tick the applicable statement:

□ I have a right to live and work in the UK indefinitely, and have supplied a set of documents 
     from List A

□ I have the right to work in the UK for a specified period of time, and have supplied a set of
     documents from List B

□  As I am from one of the A8 countries, I have complied with Section C
     (A8 counties are Latvia, Slovakia, Czech Republic, Lithuania, Slovenia, Estonia, Hungary 
     and Poland)

□  As I am Bulgarian or Romanian, and am not a student, I have complied with Section D

I confirm I understand if I do not provide the appropriate documentation, I will not be able to continue in my employment. If I have a right to work which expires and I fail to provide a new document proving my right to work in the UK, my employment will not be able to continue. I hereby give permission for Armstrong Appointments to contact the United Kingdom Border Agency or any other Government Body to verify my Immigration status and eligibility to work in the United Kingdom.
